- Patient Declaration Form - Must be signed with the patient's own handwriting signature as shown in the Emirates ID. (Attached)
- Valid Passport Copy - Scanned in PDF file format (not photographed), clear and colored copy.
- Valid Emirates ID Copy - Scanned in PDF file format (not photographed), clear and colored copy.
- Valid Residence Visa Copy - The residency must be not less than 6 months within UAE. If the current visa is issued less than 6 months, please provide a copy of your previous visa to verify the 6 month residency. Note: You can upload a clear PDF copy of your Visa through the UAEICP Mobile App: https://icp.gov.ae/en/services/icp-mobile-app/
- Latest Complete Medical Summary - Medical reports from the date of diagnosis, surgery reports, imaging results, lab test findings, pathology reports and the latest medical report with a treatment plan to complete your treatment (if possible).
- Latest Medicine Prescription - With the name of medicine [Trade name & active ingredient, dosage, frequency time and route].
- Valid Copy of the Health Insurance Card -Â Scanned in PDF file format (not photographed), clear and colored copy.
- Full Treatment Cost Estimate - We require cost estimates for medicines, administrative fees, lab tests, and any other services necessary to complete the full treatment. This quotation should be issued within the last two months, clearly indicating the date, signature, stamp, and the patient's MRN or file number.
- Patient's Salary Certificate -Â Â Must be issued not later than 2 months - Scanned in PDF file format (not photographed), clear and colored copy.
- Patient's 6 months Bank Statement - Must be 6 months until the date of application - Scanned in PDF file format (not photographed), clear and colored copy.
Terms:
* Please note the application will be examined only after submitting all required documents (In case if any of the documents were not provided or not uploaded in PDF format, the application will not be accepted).
* If there are any changes in your current status, it is important to inform Rahma about these changes and upload updated documents to your online file.
* The application will undergo a review and scrutiny process for a period of time and accordance to the Charity mechanism. Rahma is not liable to provide any justification for the applicant in case if the application was rejected.
