We require a few documents to begin the application process.

Patient Declaration Form (to be filled up and signed by the patient only) - click to download

Residence Visa (Patient must be a UAE resident for at least 6 months)

Family Member’s Residence Visa (only for unmarried – 18 years old and above if living in the UAE)

Family or Sponsor Declaration Form (to be filled up and signed by family or sponsor only) - click to download‍

Latest Medical Report (within 2 months with date, signature, stamp, and MRN or file number)‍

Latest Medicine Prescription (with the Doctor’s signature, date, and stamp)‍

Valid Quotation of the treatment or medicine (with signature, date, and stamp)‍

Patient’s latest salary certificate (if employed)‍

Latest salary certificate of all the employed family members (within 2 months)‍

6 months Bank Statement of the patient with a stamp (if no bank account, please write in the Declaration Form)‍

6 months Bank Statement of all the family members with a stamp (if no bank account, please write in the Declaration Form)‍

Latest Tenancy Contract or Title Deed (only if the property is under the patient’s or sponsor’s name)‍

Individual Credit Report from Al Etihad Credit Bureau (if the patient is employed and for the employed family) - click here to apply.
